Very interesting case with a happy end. Why you did not try a single catheter approach for coronary angiography.... for example a Tiger Catheter?

This is a catastrophic situation that was managed by excellent interventionist. I have never faced to this case so far. I think we should follow up this patient more tightly.

You could avoid this complication using a single cathether like multipurpose. And you must use a guidewire to change your catheter, independent if you use radial or femoral access, it is basic. But you solved the problem very well, congratulations.

I always exchange catheters with a magic torque by radial approach,but sometimes is difficult to advance at brachial artery.Even after good verapamil plus nitro and heparin doses.perhaps,now that we have the monday s newspaper and after knowing the soccer play result,would be interesting to pass with a 0.014 coronary wire,and make an angiographic test with the sheath.Not loosing the position,anyway an excellent case,we learned a lot and thank you so much to share with us.really
