About long inflations we must say that they can provoke ischaemia. Do you remember perfusion balloons? They are no more on the market. They were very useful in the prestent era when we wanted to seal coronary dissections and avoid surgical switch of the patient. Perfusion balloon have holes proximally and distally to the balloon so distal flow was possible even during the long inflation periods. I think tath perfusion balloons could be useful even today in occasions similar to your brilliant case.
