Great Case, Great learning points, Have had to face same complication and also once with LMS. One option which may be considered that if your proximal RCA wire is in true lumen rather than waiting for the CTO segment to be tackled you may first secure the RCA ostium and thus seal the point of entry. This way decompensation chances are reduced and if not on that attempt you can come back later and re attempt. As far as follow up -- perfectly done. Once again congratulations.
