  1. Left ventricle apex hypoplasia, with single left coronary artery (and origin of right coronary artery from mid left anterior descending)
  2. Single left coronary artery and origin of right coronary artery from mid left anterior descending, which is acutely occluded just below the bifurcation
  3. Anomalous right coronary artery origin from left circumflex and separate origin of left anterior descending (not shown)
  4. Collateral circulation from left coronary artery to right coronary and chronic total occlusion of left anterior descending
