- Left ventricle apex hypoplasia, with single left coronary artery (and origin of right coronary artery from mid left anterior descending)
- Single left coronary artery and origin of right coronary artery from mid left anterior descending, which is acutely occluded just below the bifurcation
- Anomalous right coronary artery origin from left circumflex and separate origin of left anterior descending (not shown)
- Collateral circulation from left coronary artery to right coronary and chronic total occlusion of left anterior descending
