Epicardium shadow are abnormal . It seems like coronary strangulation by myocardial prolapse out of epicardium or compression .

Probably iatrogenic massive air embolism into left coronaries causing ST elevation and finally arrest! This is a situation that should be treated by forcing great amount of saline intracoronary while supporting patient with saline iv and probably vasopressors!
