Summary

This PCR Tokyo Valves 2025 session explores the latest insights into TAVR explantation. It begins with the management of patients with longer life expectancy, then delves into surgical aortic valve replacement, aortic root enlargement, and commissural alignment—key techniques that may improve reintervention outcomes. The session also examines reintervention strategies for SAVR failure and highlights the knowns and unknowns of TAVI-in-TAVI outcomes, all with the goal of refining the concept of lifetime management.

Learning Objectives

  • To understand the key concepts behind the lifetime management of aortic stenosis
  • To learn how to optimise the initial valve choice, factoring surgical risk, life expectancy, valve durability and haemodynamics
  • To understand the key concepts in evaluating redo TAVI feasibility
  • To understand the key advantages and disadvantages of reintervention methods after an index TAVI or surgical aortic valve replacement

Presentations available when logged in:

  • Welcome and session objectives
  • A patient with a longer life expectancy: how should we treat this patient?
  • Technique to improve the outcomes of reintervention: surgical aortic valve replacement and aortic root enlargement
  • SAVR failure and reintervention options
  • Techniques to improve the outcomes of reintervention: commissural alignment
  • Outcomes of TAVI explant: is it really bad as reported?
  • Outcomes of TAV-in-TAV: the knowns and the unknowns
  • Putting it all together: what is the lifetime management?
