59-year-old female with a history of percutaneous closure of an ASD type 2 was referred for a TEE to investigate a possible cardioembolic cause of a middle cerebral artery stroke. The TEE revealed a small membranous VSD, a small atrial shunt adjacent to the AV valve plane, and an unusual mitral valve.
