A 66-year-old man with a history of hypertension, dyslipidemia and previous surgical mitral commissurotomy presented with hypotension and sweating during hospitalization for an ischemic ictus. The electrocardiogram showed ST elevation in lateral leads, and the left ventricle was globally hypokinetic on echocardiogram; therefore, the patient underwent urgent invasive coronary angiography.
