A 71-year-old male complained of bilateral leg swelling and dyspnea for one year. Echocardiography reported elevated right ventricular systolic pressure at 40 mmHg. Transthoracic echocardiography misinterpreted the abnormal aliasing around the atrial septum as an atrial septal defect. Transesophageal echocardiography provided further information.
