An 87-year-old man underwent staged PCI for a residual LAD lesion after successful primary PCI for RCA infarction. OCT showed a lipid-rich plaque in proximal LAD and severe calcification in mid-LAD. After IVL-DES in mid-LAD and scoring balloon + DCB in proximal LAD, OCT revealed an unexpected protruding structure in proximal LAD.
