Temporary pacemaker (TPI) lead was inserted through right femoral vein into right ventricle in 72-year-old male with complete heart block. Capture loss was noted on day 3. CT scan chest (Figure A, B), coronary angiogram (figure C) and thoracotomy (figure D) were done.
