Summary

Watch the case of a 69-year-old male patient with atrial fibrillation, peripheral artery disease and severe pulmonary disease, referred for worsening angina. The angiography revealed a tight distal left main stenosis on a trifurcation.

In this case, the PCI is guided by IVUS with provisional stenting (LM-LAD) and protect wires in the ramus and in the circumflex, followed by a POT, then re-wired of the ramus, kissing and final POT.
