Clinical presentation

This is an 80-year-old female whose cardiovascular risk factors include hypertension and hyperlipidemia.

Case Management

She had an ACS and a coronary angiogram showed 3 vessel disease. An echocardiogram showed EF 50%. Given her age and limited mobility, she was deemed too high risk for CABG. Accordingly, she was offered high risk multivessel PCI. Intracoronary imaging with IVUS showed distal LM and ostial LAD disease requiring a 2 stent strategy (DK Crush).
