Clinical presentation
- 75-year-old male
- Out-of hospital resuscitation, bystander witnessed
- Acute inferior-lateral MI treated with PCI of RPLD and RCX
- Staphylokokkus aureus pneumonia
- Mild to moderate neurological impairment
After heart team discussion, the patient was planed to PCI of LM/LAD and RCX before discharge to neurological rehabilitation.
Initial angiography
After primary PCI of RCA and distal RCX
Procedure
Pre-dilatation with NC-balloon and angiosculp.
Angiosculp could not be removed from left main

SB wire removed, deep guide intubation to reduce friction in the attempt to remove the balloon
Tear-off of the Angiosuclp
Detection of the distal part of the sculping balloon catheter in the distal guide catheter
Final angiographic result
Successful stent implantation in the left main.
