A 61 year old patient with a severely stenotic bicuspid aortic valve received a pre-operatory coronary angiography. Despite numerous efforts, it was not possible to visualize the origin of the left coronary at the left ostium. Catheterization of right coronary artery (RCA) demonstrated a normal implantation at the right sinus.
