A 65-year-old woman with a history of diabetes, hypertension and smoking, presented a 2-day history of severe chest pain. EKG showed septal necrosis and anterolateral ST segment elevation. Under presumed diagnosis of ST segment elevation myocardial infarction (STEMI), she was transferred to the cardiac catheterization laboratory, painless. Occlusion of a diagonal branch and proximal circumflex lesion were identified. Percutaneous coronary intervention of the circumflex artery was performed. She was transferred to an intermediate care unit. Twelve hours after the procedure, the patient referred recurrent chest pain, a new ST segment elevation was observed. Coronary angiography was performed, verifying patent stent.
