Our patient was implanted with a Zotarolimus stent 7 years ago and presented with recurrent acute ACS secondary to very late stent thrombosis.On each occasion, he presented to the peripheral hospital with acute STEMI, and was thrombolysed.Initial CAG didn't show abnormalities.

He appears to have some reaction to the stent with ectasia/remodelling outside the stent and the stent is left with free floating struts that are obviously thrombogenic. ? reaction to polymer or drug
