A 65-year-old patient presented with atypical symptoms and ST-segment depression on stress ECG.Sudden in-hospital cardiac arrest with eCPR with veno-arterial ECLS and due to increased afterload from retrograde aortic flow and lack of left ventricular (LV) unloading (no pulsatility) --> LV distension --> start with pLVAD in ECMO-Combination for LV unloading (ECMELLA venting); no coronary artery disease (LV EF 25 %)
