Inconspicuous yet sinister cause of angina in a young male

A 41-year-old male was rushed into Emergency care by paramedics for acute onset classical angina after a gym workout. He experienced ongoing chest pain and his presenting ECG was grossly abnormal showing Giant T inversions (Dagger T’s) with down-sloping ST segments.
Urgent angiogram performed was normal and bedside ECHO showed gross LVH but preserved EF. Apical views were not clear.
