A 73-year-old woman with obesity, high blood pressure and atrial fibrillation (AF), went to the hospital for worsening of tiredness for small efforts and edema in the lower limbs for 2 weeks.Cardiac auscultation revealed grade 2 continuous murmur in the left edge of the sternum and pulmonary auscultation bibasal crackles.Blood sample analysis had increased levels of brain natriuretic peptide and the ECG showed AF, 75 bpm.
