A coronary angiography was performed on a 76-year-old male with NSTEMI. LAD showed a subocclusive lesion, so angioplasty was made. When carrying out angiography of the right coronary, a dissection of its ostium with rapid extension was observed.

Next step? Immediate surgery?
Iatrogenic aortic dissection Dunning class 3. Stenting of the RCA to seal the entry site may solve the problem. However, surgery may be needed if there is further extension or aortic regurgitation.
