An 80-year-old woman with a surgical bioprosthetic aortic valve was admitted for intermittent chest pain. The initial ECG showed an ST-elevation acute anterior myocardial infarction in sinus rhythm.
The patient underwent a chest and abdominal CT angiography to rule out acute aortic syndrome. Coronary angiography subsequently revealed a “horseback” thrombotic subocclusion at the bifurcation between the left anterior descending artery and the first diagonal branch.
