An acute coronary syndrome with severe global myocardial ischemia 6 months after TAVI

A 76-year-old lady presented 6 months following TAVI (Evolut FX 26mm) with recurrent syncopes, no chest pain or dyspnoea. ECG showed global myocardial ischemia. There were new regional wall motion abnormalities in the LAD territory, with no evidence of prosthetic valve dysfunction on ECHO.
