A 70-year-old patient with a history of chronic coronary syndrome, heart failure, and CRT-D was admitted for angioplasty of in-stent CTO. The artery was recanalized, and multiple passes of a 1.7 mm excimer laser catheter were performed. Based on IVUS findings, sequential balloon predilatation, including DCB, was performed. IVUS was used to confirm lumen enlargement.
