In a routine PTCA, LAD artery was dilated with 2.5x15 mm semi-compliant balloon. Angiogram immediately after it showed Ellis’s grade III perforation. However, there was no pericardial effusion or any immediate hemodynamic compromise.

    According to this image the intramyocardial perforation was occur, in the muscular bridge segment. The perforation to right or left ventricle can occur. However according to this image we can not say the perforation to coronary sinus.
