Aortic root illusions

A 26-year-old male presented with progressive dyspnea on exertion (NYHA II) for 2 months. Examination revealed a markedly wide pulse pressure with diastolic blood pressure of 46 mm Hg and no blood pressure difference between all four limbs. No history of infective endocarditis, hypertension. Multiple peripheral signs of severe aortic regurgitation were present. Transthoracic echocardiography showed severe aortic regurgitation with a mobile linear echogenic structure in the aortic root, intermittently prolapsing into the LVOT and left ventricular cavity, raising suspicion of an aortic root dissection flap.
