What mechanism explains “the dark side” of this late stent failure?

A patient with prior coronary stenting presents for evaluation of late stent failure. OCT reveals a large signal-poor region between 3 to 4 o’clock confined within the stent. Although the luminal compromise is not much, the presentation is an acute coronary syndrome.
