An 81-year-old lady with past history of surgical aortic valve replacement followed by implantation of dual-chamber permanent pacemaker was admitted for recurrent episodes of right heart decompensations. The echocardiographic evaluation revealed massive TR and moderate primary MR due to a retracted posterior leaflet. What would be your treatment strategy?
