A 66-year-old male patient complained of exertional dyspnea on minimal exertion. He was a heavy cigarette smoker with hyperlipidemia, but not hypertensive or diabetic. Echocardiography showed no significant abnormalities, ECG was normal, exercise tolerance test very positive.
