Summary

Learn from these 2 cases of primary mitral repair using percutaneous and surgical approaches:

  1. LIVE case - A 90-year-old male with history of LAD PCI 23 years ago and paroxysmal atrial fibrillation presents a primary mitral regurgitation (P2 prolapse) on mildly dilated LV and preserved LV function.
    The operators implanted 2 G4 clips: first XTW and second XT to stabilize the result on general anaesthesia, TEE guidance and left atrial pressure monitoring.
  2. Recorded case - A 73-year-old male with a barlow MR, has a history of transapical 3 Neochords (2019), 3 clips (2020), 2 clips (7/2022), with still a severe MR and a new severe TR.
    The operators explant surgically the 5 clips, perform a surgical mitral repair (mitral ring + mitral repair) and implant a tricuspid ring.
