Hybrid procedure: Stent and DCB for a long diffuse lesion - LIVE case
Stent and paclitaxel DCB on mid and distal LAD guided by OCT
Summary
A 40-year-old male with no comorbidities presented an NSTEMI. The angiography showed a focus lesion on the mid-CX and a diffuse lesion on the mid and distal LAD. The LAD FFR are 0.49 with a diffuse pullback gradient.
After Stent in mid-CX guided by OCT, the operators implanted first a long stent on the mid-LAD with post-dilatation, and then used 2 DCB for the distal and very distal part of the LAD. Watch the procedure!

Key moments
- 6:20-9:03 - Angiography analysis
- 22:00-33:33 - OCT runs
- 58:54-1:00:54 - DCB dilatation
Keywords: Multivessel disease, OCT, Paclitaxel DCB
Learning Objectives
- To know which de novo lesions can be considered for DCB
- To learn how to optimally prepare the lesions before applying DCB treatment
- To understand when to choose DCB-alone treatment or a hybrid procedure
