A 64-year-old male patient, active smoker (1 packet daily), presents with worsening angina. Initial angiogram from the referring hospital showed a lesion in the proximal RCA, which warranted further OCT investigations to identify the nature of the lesion. OCT performed at our institution reveals the diagnosis.
