Summary

Watch the case of a 75-year-old male patient with history of TIA, symptomatic severe aortic stenosis with RBBB, and preserved LV function. The heart team's decision was to perform TAVI with PCI on LAD a few days before procedure.

First, a cerebral protection device (Sentinel) was implanted. Then, a Sapien 3 26 mm + 2CC were implanted without predilatation, and pacing was performed on safari wire, as well as placement of a temporary pacemaker lead (presence of an RBBB). All steps are clearly described in this video!

During the session, there is also an excellent example of TAVI surgical explantation.

Learning Objectives

  • To appreciate the clinical evidence governing the application of TAVI in the lifetime journey of younger patients with aortic stenosis
  • To understand the importance of clinical and anatomical factors in determining the choice between TAVI and surgical aortic valve replacement
  • To learn how valve design and implantation technique can influence long term outcomes after the index TAVI procedure

Presentations available when logged in:

  • Durability of surgical and transcatheter valves: what do we know so far?
  • Imaging is key to initial procedural strategy: getting it right first time
  • Patient presentation
  • Imaging analysis
  • Procedural "sequencing" options: when the patient outlives the valve
  • Surgery for TAVI failure: what do we know about device explantation?
