POPULAR TAVI - Aspirin with or without clopidogrel after TAVI

Reported from ESC Congress 2020 - Mirvat Al Asnag interviews Anna Sonia Petronio (Pisa, Italy) on the results of POPULAR TAVI which showed that in patients not taking oral anticoagulation, aspirin alone compared with aspirin plus clopidogrel following TAVI significantly reduced bleeding rates and did not increase thromboembolic events.

Prof. Petronio answers the following questions:

  • What are the differences between cohort A and cohort B?
  • Without imaging, subclinical leaflet thrombosis is undetectable. Do you believe this needs to be addressed in another RCT?
  • There’s concern about new AF (occurring in 20%) and young patients needing PCI and eventual crossover of those on aspirin alone. Please comment.
  • Can these results be applied to bicuspid & Valve in Valve?
  • Finally, this population is also at bleeding risk & aspirin alone strategy is shown to be safe. Would the same conclusion apply to other antiplatelet such as clopidogrel?
