Left main and multivessel disease

Latest left main and multivessel disease content in interventional cardiology - research, practice, and education

The left main coronary artery provides the primary source of blood flow to the left ventricle. Diseases affecting the left main, by itself or in combination with multivessel disease, place the patient at significant risk for life-threatening cardiovascular events. Treatment requires an understanding of all available tools, devices and techniques, like those employed in invasive imaging and functional assessment, as well as taking into account underlying comorbidities such as diabetes. Advances in imaging techniques, as well as the use of adjunctive pharmacotherapy, vascular access or invasive interventions such as bypass surgery, are important, as are understanding bleeding risks. It is a calcified left main: do it well, do it safely

28 Feb 2026 – From Euro4C Annual Meeting 2026

This session addresses the complexities of percutaneous coronary intervention (PCI) in calcified left main coronary artery disease. It covers comprehensive planning including mechanical circulatory support, intravascular imaging, and bifurcation strategies. Attendees will also learn to anticipate and manage potential complications and apply various tools to ensure...

Impact of complete revascularisation in relation to left ventricular function in patients with ST-segment elevation myocardial infarction and multivessel disease: a post hoc analysis of the COMPLETE randomised trial

05 Feb 2026

This study represents a prespecified subgroup analysis of the COMPLETE randomised trial, a large, international, randomised controlled study comparing a strategy of complete revascularisation with culprit-only percutaneous coronary intervention (PCI) in patients presenting with ST-segment elevation myocardial infarction (STEMI) and multivessel coronary artery disease (MVD).

The present...

Provisional stenting for a distal left main stenosis - LIVE Case

10 Dec 2025 – From GulfPCR-GIM 2025

A 66-year-old male with preserved LV function, type II diabetes, and hypertension, presented with CCS and anterior ischemia, with a normal ECG. Photon-counting CT revealed a significant calcified stenosis of the distal left main and proximal LAD, confirmed by coronary angiography.

A stent was implanted in the...

Provisional stenting with a scaffold stent and DCB in calcified left main bifucation - LIVE Case

11 Oct 2025 – From AICT-AsiaPCR 2025

A 59-year-old male presented with CCS for three months, with diabetes and normal LV function (72%). Coronary angiography showed a calcified left main stenosis (Medina 1,1,1) and diffuse calcification of the proximal and mid LAD. The LCX QFR was 0.86.

Treatment included extensive lesion preparation with lithotripsy...

Left main PCI - LIVE Case

10 Oct 2025 – From AICT-AsiaPCR 2025

A 78-year-old male with a history of LAD PCI in 1997 presented with an NSTEMI three months earlier due to diffuse three-vessel calcified disease, including: distal LM stenosis, severe mid and distal disease of a dominant RCA, and LAD in-stent restenosis. LV function was normal. He...

Large thrombus burden management in a young patient with inferior STEMI and multivessel disease

08 Oct 2025

A 51-year-old man with an inferior STEMI and multivessel disease was found to have a thrombotic sub-occlusion in the mid RCA. Angiography also revealed significant disease in the LAD. How would you manage this case?
